    Anglican Schools Commission Collection Notice:  

    1. By completing this form you are submitting personal information (including potentially sensitive information) to the Corporation of the Synod of the Diocese of Brisbane (Anglican Church Southern Queensland or ACSQ) through its Anglican Schools Commission (ASC).
    2. The primary purpose of collecting this information is to enable the ASC to manage, investigate and administer your complaint, feedback or protected disclosure.
    3. A protected disclosure will be dealt with in accordance with the Whistleblower Policy for Anglican Education.
    4. The ASC collects, uses, holds and discloses personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
    5. Laws governing or relating to the operation of schools also require certain information to be collected and disclosed. These laws include relevant Education Acts and Public Health and Child Protection laws.
    6. If the ASC does not obtain personal information from you, it may not be possible for us to properly investigate and manage your complaint, feedback or protected disclosure. Of course, you always have the option to remain anonymous.
    7. If you provide the ASC with personal information of others, we request that you inform them that you are disclosing that information to the ASC and why.
    8. Information may be shared with others for legal, compliance and/or insurance purposes or as otherwise required by law.
    9. The ASC may use technology service providers including cloud service providers to store personal information.
    10. The ACSQ Privacy Policy:
    11. sets out how you may seek access to or correction of your personal information held by the ASC; and
    12. how you may complain about the ASC’s management of your personal information.
